REMINDER - Car Park Closure - Due to essential renovation works at the surgery, the rear car park is closed until further notice. Update Your Details

This section is for patients already registered with Clifton Road Surgery and not part of the registration process.

It is important to update your details like your name, address, phone numbers or email address. Please note if the updates are relevant for more than one person in the household you must complete an admin request form for each individual.

Simply click on this link >>> Change of Details Request. On the new screen that will pop-up select the “I have an admin or routine care request” option; then click on “Other admin request”. Please then let us know of your update/s – please remember a form will need completing for everyone in your household if the update/s also apply to them.
